LDL'nin damar intima matrisinde oksitlenmesi (oxLDL) aterom oluşumunun ilk adımıdır. Endotel hürlerinin oxLDL tarafından uyarılması kandaki monositelerin seferber olup damar duvarına girmesine neden olur. Monositlerin makrofajlara dönüşümü ve kümelenmiş oxLDL'nin Avcı Reseptör (Scavenger receptor') tarafından bu hücrelerin içine alınması sonucu köpük hücreler meydana gelir. Damar düz kas hücrelerinin uyarılmısı bunların harekete geçmelerine ve çoğalmalarına neden olur.
